PERSONAL DATA PROTECTION LAW APPLICATION FORM
APPLICATION FORM as per the Articles 11 and 13 of THE LAW ON PROTECTION OF PERSONAL DATA
The personal data owners defined as the person concerned in the Personal Data Protection Law No. 6698 (hereinafter referred to as the "Applicant") have been granted the right to make certain requests regarding the processing of their personal data in Article 11 of the Law.
Pursuant to the first paragraph of Article 13 of the Law, the applications to be made to our data supervisor company regarding these rights must be submitted to us in writing or by other methods determined by the Personal Data Protection Board.
Your application and the request in its content are concluded free of charge by GIZIA, the data supervisor, as soon as possible and within thirty days at the latest, depending on the nature of the request. However, if the process requires an additional cost, a fee may be charged according to the tariff determined by the Board[1]. Your request will be accepted or rejected with justification within this period, and your response will be notified to you in writing or electronically.
The information and documents you have provided in the application form will be processed by GIZIA within the scope of the clarification text available at https://www.gizia.com/kvkk, limited to the purposes of evaluating your application, answering it, delivering the answer to you, determining your identity and address.

[1] Pursuant to the Article 7 titled "Fee" of the Communiqué on Application Procedures and Principles for the Data Supervisor, “(1) If the application of the relevant person is to be answered in writing, no fee is charged for up to ten pages. A transaction fee of 1 Turkish Lira may be charged for each page above ten pages. (2) If the answer to the application is given in a recording medium such as CD or flash memory, the fee that may be requested by the data supervisor cannot exceed the cost of the recording medium"
